Abstract:
This Undergraduate Thesis addresses “The role of the clinical pharmacist in the
hospital setting and their responsibility in preventing medication errors” and aims to
analyze the applicability of this professional’s duties in reducing medication errors in
hospital environments, based on protocols and clinical practices established between
2021 and 2025. This is a bibliographic research with a qualitative and descriptive
approach, structured to examine evidence published in national scientific journals
regarding the activities and responsibilities of the clinical pharmacist in the hospital
context. The results demonstrated that the clinical pharmacist plays an essential role
in preventing and mitigating medication errors, establishing themselves as a key
agent in patient safety. Among their main actions are the prospective validation of
prescriptions, medication reconciliation, standardization of operational protocols, and
continuous education of multiprofessional teams. The constant presence of this
professional in critical sectors such as emergency care, intensive care, and oncology
was shown to reduce therapeutic discrepancies and improve the quality of care,
ensuring greater treatment effectiveness and minimizing clinical risks. The reviewed
studies also reported high acceptance rates of pharmaceutical interventions, ranging
from 76% to 96%, reflecting technical credibility and recognition of the pharmacist’s
importance within the healthcare team. In addition to technical activities, their
educational and managerial role was highlighted, contributing to an organizational
culture focused on safety, traceability, and process standardization. It is concluded
that the clinical pharmacist’s role transcends medication dispensing, assuming a
strategic function in managing medication-related risks and in hospital governance,
thus strengthening safer, more efficient, and more humanized healthcare systems.
