Abstract:
This study aims to report a clinical case of aesthetic and functional treatment of a
gummy smile through gingivectomy associated with osteotomy, highlighting the
importance of individualized diagnosis and treatment planning. This is a descriptive
case report study conducted on a 26-year-old male patient treated at a dental clinic,
whose main complaint was related to smile aesthetics. Clinical examination, detailed
anamnesis, periodontal probing, photographic records, and periapical radiographic
examinations were performed to define the diagnosis of altered passive eruption. The
treatment consisted of clinical crown lengthening through gingivectomy associatedwith osteotomy, with subsequent postoperative follow-up. The results demonstrate a
significant improvement in smile harmony, with adequate tooth exposure, favorable
gingival contour, and patient satisfaction, without clinical complications and with
maintenance of periodontal health. It is concluded that the association between
gingivectomy and osteotomy constitutes an effective, safe, and predictable approach
for the treatment of gummy smile in cases of altered passive eruption, providing
aesthetic and functional benefits, as well as a positive impact on the patient's
self-esteem.
