dc.description.abstract	The role of clinical pharmacists in hemodialysis clinics has proven essential for promoting patient safety, optimizing pharmacotherapy, and strengthening interdisciplinary practices. This study aimed to analyze the perceptions of themultidisciplinary team regarding the importance of including clinical pharmacists in a hemodialysis clinic located in the Triângulo Mineiro region of Minas Gerais. The research was quantitative and descriptive in nature, conducted through the application of a structured questionnaire to 21 health professionals, including physicians, nurses, nursing technicians, nutritionists, psychologists, and social workers. The results showed that most participants recognize the importance of the pharmacist's presence to ensure the rational use of medications, reduce adverse events, and contribute to patient safety. The responses also showed a positive perception of the integration of pharmacists into the team, reinforcing the need for institutional policies that ensure their effective performance in the context of renal replacement therapy. It is concluded that the inclusion of clinical pharmacists in hemodialysis clinics represents an advance in the quality of care and the humanization of care, promoting direct benefits to both patients and the multidisciplinary team.	pt_BR

dc.description.resumo	A atuação do farmacêutico clínico em clínicas de hemodiálise tem se mostrado essencial para a promoção da segurança do paciente, otimização da farmacoterapia e fortalecimento das práticas interdisciplinares. Este estudo teve como objetivo analisar as percepções da equipe multiprofissional acerca da importância da inserção do farmacêutico clínico em uma clínica de hemodiálise localizada no Triângulo Mineiro, Minas Gerais. A pesquisa foi de abordagem quantitativa e descritiva, realizada por meio da aplicação de um questionário estruturado a 21 profissionais da área da saúde, incluindo médicos, enfermeiros, técnicos de enfermagem, nutricionistas, psicólogos e assistentes sociais. Os resultados demonstraram que a maioria dos participantes reconhece a relevância da presença do farmacêutico para garantir o uso racional de medicamentos, reduzir eventos adversos e contribuir para a segurança do paciente. As respostas também evidenciaram uma percepção positiva quanto à integração do farmacêutico à equipe, reforçando a necessidade de políticas institucionais que assegurem sua atuação efetiva no contexto da terapia renal substitutiva. Conclui-se que a inclusão do farmacêutico clínico nas clínicas de hemodiálise representa um avanço na qualidade assistencial e na humanização do cuidado, promovendo benefícios diretos tanto aos pacientes quanto à equipe multiprofissional.	pt_BR
